Responsibilities

• Educates members and families about program services, ensuring understanding of the program, benefits and expectations.
• Collaborates with youth and families to identify immediate needs, strengths, and concerns, and make appropriate referrals.
• Completes intake processes, assesses members’ needs, maintains a caseload, and inputs documentation into databases.
• Provide direct clinical consultation and guidance to Care Coordinators managing complex, high-acuity cases.
• Facilitates clinical staffing meetings to ensure safety planning and service delivery.
• Guide staff through processes to explore funding for residential treatment.
