Job Description
The Case Therapist I reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsible for assessing patients for acute mental health crisis and substance concerns in emergency department and medical/surgical units, treatment planning, providing direct patient services, including individual and family counseling if needed. Case Therapist coordinates care including communicating with treatment team, patient, and family, documenting in medical record, and discharge planning, continuum of care.
Hours: 3pm-11:30pm, including a weekend rotation of every third weekend.
