  • 37.5-hour work week
  • M – F, 7.5 hour shifts, between the hours of 8:30 am – 4:30 pm (some flexibility required) Must be able to work remotely, in office, and in the community.

JOB DESCRIPTION: A Crisis Case Manager (CCM) is responsible for assisting individuals with mental illness who reside within the community and are in need of psychiatric rehabilitation to access acute care resources. The role of the CCM is to maintain and enhance the functioning level of this population to establish healthy, constructive living patterns and reduce the rates of recidivism into acute services. The CCM is expected to work, with a high degree of independence, to address the needs of individuals with mental illness by providing advocacy, monitoring, linking, and support services.

SPECIFIC RESPONSIBILITIES INCLUDE:

  • Serves as an advocate, facilitator, monitor, supporter and primary resource for assigned clients.
  • Implements the principles of psychiatric rehabilitation by encouraging the empowerment of each assigned client.
  • Conducts individual needs assessments.
  • Encourages client participation in appropriate programming services and provides assistance as needed.
  • Assists clients in exercising better judgment skills through supportive monitoring.
  • Supports and encourages clients as they attempt to adjust to new services.
  • Links clients by arranging appointments with service providers and facilitating the receipt of such services, as well as arranging for transportation when appropriate.
  • Reports crisis situations and collaborates with Crisis Counselors when set-backs indicate the need for assessment and/or stabilization.
  • Conducts outreach visits to high-risk individuals, with or without law enforcement assistance.
  • Conducts face-to-face visits with patients being discharged from Emergency Departments and inpatient psychiatric units and provides intensive post discharge follow- up contact.
  • Provides outpatient visit reminders for clients and follow- up.
  • Provides linkage for clients to formal and informal support networks.
  • Collaborates with service providers to expedite access to services for clients.
  • Builds and fosters the development of collaborative relationships with community service providers.
  • Works in partnership with Mental Health Court to engage persons in services and educate on appropriate crisis services.
  • Accompanies Crisis Counselors on assessments where a joint response is indicated due to safety or other concerns.
  • Maintains self in professional manner at all times, adhering to Neighborhood Center policy and procedures and in keeping with the Agency’s mission statement.
  • Performs other duties as assigned including but not limited to assignment to the 911 Call Center to triage crisis calls.
  • Punctual and regular attendance is an essential responsibility of each employee.
  • Employees are expected to report to work as scheduled, on time and prepared to start
    working.

REQUIRED QUALIFICATIONS:

  • Experience with and knowledge of the mental health delivery system and community resources.
  • Work experience demonstrating organizational, assessment and communication skills commensurate with this position.
  • Experience in securing the cooperation of and working effectively with other professionals and agencies in the community.
  • A Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field with a minimum of three (3) years experience working in a human services agency.
