Job summary

Bay Cove Human Services is seeking an RN/LPN to provide direct nursing care on the CCS unit, promoting safety, health, and crisis resolution for clients. The role coordinates with Cape Cod Community Behavioral Health Center to ensure medication continuity and smooth transitions to community-based care.

The position requires MA RN or LPN licensure, 3+ years in behavioral health, CPR certification, and strength in team collaboration and crisis management.

Qualifications

• Massachusetts Licensed RN or LPN with a minimum of 3 years in behavioral health nursing.
• Current CPR certification.
• Ability to work collaboratively in multidisciplinary teams.
• Ability to perform under pressure with clarity, compassion, and sound judgment.
• Strong problem-solving skills and initiative.
• Ability to work independently.

Responsibilities

• Provide direct nursing services to CCS clients including medication administration and health assessments.
• Coordinate with CBHC prescribers, nursing staff, primary care partners, and pharmacies for continuity of care.
• Participate in daily rounds with a multidisciplinary team.
• Coordinate admissions and discharges to and from the CCS unit.
• Maintain accurate nursing documentation in eHana and support care-transition workflows.
• Ensure safety protocols are reviewed and followed by shift staff.
