Sea Mar Community Health Centers in Washington State is hiring a Licensed Practical Nurse for the Crisis Care Center to support clients stabilizing from behavioral health crises. The LPN will provide direct nursing care, screenings, and participate in planning with the team.

This full-time position requires WA LPN license, BLS CPR within 90 days, TB testing, and vaccination compliance. Mental health experience preferred; benefits include medical, dental, vision, 401(k), and generous PTO.

Qualifications

• Licensed practical nurse (LPN) in Washington state.
• Must complete BLS CPR within 90 days and maintain.
• Must pass pre-hire and annual TB screening.
• Mental Health experience preferred.
• Vaccines: annual influenza and Covid-19 vaccine required.

Responsibilities

• Coordinate with clients, providers, and teams to deliver comprehensive care.
• Conduct thorough assessments and maintain clinical documentation.
• Develop and update individualized care plans in response to changes.
• Ensure compliance with infection control, safety, and regulatory requirements.
