Position Summary

Provide liaison services for individuals with mental illness or co-occurring disorders who are incarcerated or at risk of incarceration in Davidson County's Criminal Justice system.

Responsibilities
  1. Identify and engage seriously and persistently mentally ill (SPMI) individuals involved in the Criminal Justice System.
  2. Identify service providers across various organizations and agencies serving SPMI consumers.
  3. Develop and coordinate community resources for this population.
  4. Educate service providers about agency programs and solicit their assistance.
  5. Assess referrals for appropriateness and immediate needs.
  6. Develop and coordinate release plans from jail.
  7. Link consumers with their mental health service providers.
  8. Follow up post-release to ensure successful community reintegration.
  9. Complete necessary paperwork, including input into the TNWITS system.
  10. Present assessment and enrollment documentation for assignment.
  11. Consult with Care Managers and providers on assessments as needed.
  12. Communicate trends and issues to program staff and community partners.
  13. Collaborate with TDMHSAS staff to develop mental health training for law enforcement and other personnel.
  14. Ensure adherence to program protocols and policies.
  15. Submit monthly and quarterly reports as required.
  16. Address team concerns promptly and seek resolution.
  17. Support Care Managers in crisis situations.
  18. Review clinical documentation for individualization and adequacy.
  19. Facilitate smooth transitions within the agency and community.
  20. Assist with referrals, linkage, and advocacy for consumers.
Other Responsibilities
  • Attend all mandated trainings and continuing education.
  • Comply with all compliance policies, practices, and procedures.
  • Follow OSHA and safety policies, reporting unsafe practices or incidents.
  • Maintain compliance with privacy and security regulations (HIPAA).
  • Participate in proactive team efforts to achieve departmental and company goals.
  • Perform other assigned duties.
  • Provide leadership through example and sharing of knowledge and skills.
Essential Skills and Experience
  • Master’s degree in counseling, psychology, social work, sociology, or related behavioral health field preferred.
  • Consideration given to Master’s in Criminal Justice or related fields.
  • Minimum of 2 years Care Management experience for internal applicants.
Essential Requirements
  • Valid Tennessee Driver’s License.
  • Acceptable Motor Vehicle Report (MVR).
  • Acceptable criminal background check.
  • Personal automobile insurance.
  • Reliable cell phone and transportation that seats 4.

Physical Demands and Work Environment

Must occasionally stand, walk, sit, use hands, reach, talk, hear, and lift up to 15 pounds. Vision abilities include close, distance, color, peripheral, depth perception, and focus adjustment. Work environment is typically moderate noise.
