Responsibilities

• Assist in planning and conducting activities for individuals and groups.
• Support communication among staff, residents, families, and external partners.
• Assist in scheduling monthly activity calendars and attendance records.
• Help implement action plans to address quality deficiencies.
• Contribute to discharge planning and development of activity care plans.
• Arrange transportation for residents for outings or appointments.
• Encourage resident participation in hobbies and reading.
• Provide necessary materials including Braille or audio books.
• Document assessments and progress notes as assigned.
• Maintain a clean, organized Activity Department.
