Job summary

BronxWorks is seeking a Full Time Professional in Bronx, NY to support outreach, referrals, intake interviews, and data reporting. The role involves collecting data from multiple systems, training staff, and contributing to grant writing and program evaluation.

  • Collect statistical information from databases to write and complete reports.
  • Train new staff to use computerized data base programs.
  • Submit reports and analyses accompanying data reporting.
  • Present data findings in internal and inter-agency meetings.
  • Participate in grant writing projects.
  • Provide quality assurance for case management input and charting.

Qualifications (education, professional certifications or licenses, experience)

  • Proficiencyin modern business communications including personal computers, electronic mail, voicemail,facsimileand copier equipment

Essential Duties and Responsibilities

  • Provide outreach and referral development services with ACS PPRS liaisons, otherBronxWorksprograms, andcommunity basedprograms
  • Respond to and triage all referral calls
  • Provide intake services for all referrals,including intake interviews and write ups
  • Collect statistical information from Contact Grids, PROMIS and CNNX and other requireddatabases to write and complete reports
  • Collect information directly fromCase Managers and Assistant Program Directorsnecessary to compile required reports
  • Extract data from data sources toidentifyimportant trends and problems
  • Trainnew staffto use computerized data base programs
  • Submit written reports and analysis thataccompanydata reporting and presentations
  • Present data findings ininternal andinter-agency meetings
  • Interact withACSon an as-needed basis
  • Participate in grant writing projects
  • Provide quality assurance for case management input and charting inclient baseddata systems including Contact Grids, PROMIS and CNNX
  • Maintain familiarity with program resources available on- and off-site
  • AttendCSmeetingfor CQI purposes tomaintainprogram standards. Must be able to report on thesemeetingand changing standards.
  • Assistwith special projects as assigned includingrepresentingagency at interagency events
  • Report to and meet with theProgram Directoron a regular basis
  • Performadditionalduties as assigned

Program Specific Responsibilities

  • Progress notes review (Contact Grid, Promis, Salesforcer and CNNX)
  • Conduct caserecord reviews and provide reports
  • Case Planner Report, verify active cases within month Tracked Children Report )
  • FASP Due and FTC Report(Verify CP responsibility and FTCtimeframe)
  • Supervisor Case Review,Close Case Report,Connections spot check
  • Staff KPI targets (email),KPI Excel Spreadsheet andPowerpoint,KPI NarrativeSubmit reportsto Program Director
  • Maintain intake, case review, group work logs
  • Assist with ordering Program supplies with Program Director.
  • Any other duties, where appropriate

  • Ability to sit, crouch and walk for extended periods of time
  • Ability to lift, pull, push, and/or move up to 60 pounds
  • Ability to bend and retrieve objects and/or documents
  • Ability to climb stairs
  • Ability to climb and stand on a15 footladder
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
