Job summary

The Commonwealth of Pennsylvania in Honesdale, PA seeks a Blended Case Manager to plan, coordinate, and manage services for assigned caseloads using recovery-oriented practices. This County Caseworker 1 role supports children, adolescents, and families through intake, service coordination, needs assessment, recovery guidance, specialized assistance, and progress monitoring.

Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

THE POSITION

Located in Honesdale, PA, Wayne County Behavioral Health & Intellectual Disabilities agency is looking for you. The Blended Case Manager is responsible for planning, coordinating, and managing services for assigned caseloads, utilizing recovery-oriented and trauma-informed practices. DESCRIPTION OF WORK

This position supports children, adolescents, and their families by guiding them through needed services and community supports.

As a County Caseworker 1, Blended Case Manager, you will perform the following duties:

• Intake Support: Gather information during initial intake to help enroll individuals in the Behavioral Health Blended Case Management Program
• Service Coordination: Work with families, providers, and the ID Supports Coordinator to link individuals with needed community services

Work Schedule and Additional Information:

• Full-time employment; 37.5 hours/week.
• Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch.
• Employee may have overnight travel or out of County travel for meetings or training.
• Employee may have irregular hours, including evenings and weekends.
• Employee works on-call on a rotating basis 24/7 once trained.

Required Experience, Training & Eligibility

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.

Other Requirements

• PA residency requirement is currently waived for this title.
• You must be able to perform essential job functions.

• You must pass a background investigation.
