Responsibilities
  • Maintain clinical records in compliance with all regulatory bodies and hospital policies.
  • Provide assessment, case management, intervention, treatment planning, and discharge planning.
  • Collaborate with internal staff and external resources for treatment and relationship building.
  • Participate in activities to ensure payment for services, including insurance and managed care.
  • Prepare reports, maintain data, and attend meetings as assigned.
Qualifications
  • Minimum of one year experience in substance use disorder treatment or two years as a social worker or mental health counselor.
  • Master's Degree in Social Work (LMSW/LCSW) or a Master's/Doctoral degree in mental health counseling (LMHC) from an accredited institution.
  • CASAC or other QHP preferred.
  • Bilingual in English/Spanish preferred.

DOC clearance required; passing and maintaining security clearances is a condition of employment.
