This role involves effective communication and collaboration with healthcare team members to ensure safe, timely patient services, and comprehensive education for patients and families. Responsibilities include interpreting complex patient needs, assessing psychosocial risks, counseling on emotional, social, and financial aspects of illness, and mobilizing community resources. The role also involves providing mental health assessments, crisis intervention, and specialized services such as child and adult abuse intervention, guardianship, foster care, adoption, and end-of-life care. The position requires leadership in crisis situations, de-escalation techniques, and counseling at individual, group, and family levels.
