Overview

This role involves effective communication and collaboration with healthcare team members to ensure safe, timely patient services, and comprehensive education for patients and families. Responsibilities include interpreting complex patient needs, assessing psychosocial risks, counseling on emotional, social, and financial aspects of illness, and mobilizing community resources. The role also involves providing mental health assessments, crisis intervention, and specialized services such as child and adult abuse intervention, guardianship, foster care, adoption, and end-of-life care. The position requires leadership in crisis situations, de-escalation techniques, and counseling at individual, group, and family levels. Additionally, it involves utilization review and discharge planning.

Minimum Qualifications

  1. Master's Degree in Social Work, Counseling, Psychology, or related field.
  2. CPR (Non-Violent Crisis Intervention) certification required within 6 months.
  3. Current licensure or license eligibility in Indiana as an HSPP, LMFT, LCSW, or LMHC.
  4. 3-5 years of experience preferred in mental health, child and family services, veteran's services, or substance abuse.
  5. Experience working with diverse patient populations.
  6. Knowledge of disease impact, human behavior, legal mandates, growth and development principles, and crisis intervention techniques.
  7. Proficiency in Microsoft Excel and Word.
