Job summary

Pinnacle Treatment Centers is seeking a Counselor to support the recovery journey of patients across our treatment network. The role emphasizes evidence‑based practices, compassionate care, and documentation oversight in alignment with federal and state standards.

You will guide patients through program objectives, supervise intake and group sessions, and participate in supervision and training to maintain high quality care.

Qualifications

• GED/High School diploma or higher.
• CDCA certification in Ohio.
• Counselors with less than one year of experience must plan supervision with their supervisor within one year.
• Counselor II requires relevant degrees and licensure in OH.
• Counselor III requires a master’s degree and independent licensure or equivalent.

Responsibilities

• Ensure completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation per federal/state standards.
• Provide orientation to patients about program objectives, rules, hours, and rights to help adjustment.
• Assess and place patients according to treatment phases and policies.
• Update clinical documentation and patient charts according to policies.
• Counsel patients in crisis situations and arrange referrals to community resources.
• Participate in case conferences and reviews and maintain confidentiality regulations.

Key Responsibilities:

• Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures.
• Provide patients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of services, patient rights, and additional information to help patients adjust to the program.
• Assure each patient is assessed and placed according to the treatment phases in accordance with Federal and State standards and agency policies.
• Assure all clinical documentation and patient charts updated in accordance with established policies and procedures.
• Counsel patients in crisis situations by identifying patient resources and coping abilities to help patients cope with and resolve crisis.
• Counsel patients who have refused to comply with specific program requirements to increase patient awareness of consequences of noncompliance.
• Arrange referrals to other agencies and resources in the community when appropriate.
• Participate in case conferences and case reviews in accordance with established policies and procedures.
• Develop and increase knowledge of federal, state, and agency regulations regarding confidentiality, methadone treatment, and substance abuse counseling certification.
• Submit all internal and external correspondence to supervisor for review and approval.
• Complete intakes, facilitate group counseling sessions.
• Collection and testing of urine/swab drug screening following proper chain of custody.
• Secure necessary documentation from patients, when appropriate, to ensure compliance with established policies and procedures.
• Submit monthly and quarterly statistical reports to supervisor.
• Attend supervisions as scheduled
• Attend team meetings and complete all training courses timely as required.
• Other duties as assigned.

Qualifications:

Counselor I

• GED/High School diploma or higher
• and CDCA certification in Ohio
• Counselors with less than one year of full-time equivalent experience in the field of addiction treatment shall develop with their supervisor a plan to achieve competency prior to providing counseling services without their supervisor present during or constantly observing counseling sessions. The plan must specify the frequency of face‑to‑face clinical supervision meetings between the counselor and supervisor, and the timeframe for achieving competency which shall be no more than one year.
