Centra Healthcare Solutions is seeking an Occupational Therapist Assistant (COTA) for a SNF/LTC setting in San Jose, CA. The role is a direct hire, full‑time position with day shifts. Responsibilities include assisting in rehabilitation plans, monitoring progress, and collaborating with an interdisciplinary team.

Required qualifications include an Associate degree or certificate, current state licensure as a COTA, and CPR certification. Prior experience is preferred but new grads are considered.

Qualifications

• Must have earned either an Associate degree from an accredited school or a certificate from a community, technical or vocational college.
• Current state license as an Occupational Therapist Assistant / Occupational Therapy Assistant / COTA or in process for licensure.
• Current CPR certification.
• Assist patients with rehabilitative activities and exercises outlined in a treatment plan developed with an occupational therapist.
• Assess the learning needs and capabilities of patients and caregivers with regards to age, level of understanding and emotional status.
• Demonstrate sound clinical reasoning in treatment planning, implementation and progress monitoring.
• Collaborate within an interdisciplinary team to meet patients' goals.
