Job summary

Blue United Sourcing is seeking experienced Certified Occupational Therapy Assistants (COTAs) for a Skilled Nursing Facility in San Jose, CA. Positions are open to travel, permanent, or travel-to-perm candidates with ASAP start.

Two openings available in Silicon Valley region. Role involves OT evaluations, treatment planning, and collaboration with interdisciplinary teams to support residents’ ADLs and mobility.

Qualifications

• Current COTA license in California.
• Associate’s or higher degree in Occupational Therapy Assistant from an accredited program.
• Ability to follow OT treatment plans and assist with interventions.
• Strong communication and teamwork skills for interdisciplinary collaboration.
• Accurate documentation and time management skills.
• Commitment to following facility policies and state regulations.

Responsibilities

• Assist with evaluations and implement OT treatment plans.
• Provide therapy to improve residents’ ADLs and functional mobility.
• Support interventions for orthopedic, neurological, and post-acute conditions.
• Teach adaptive techniques and proper use of assistive devices.
• Collaborate with OTs, nursing, and rehab teams on care goals.
• Document therapy sessions, progress, and daily notes accurately.
• Monitor resident responses and report outcomes to supervising OT.

• Salary range: Up to $1,800 weekly
• Anticipated duration of contract: 13 weeks
