Job summary

Home Health Care, Inc. is seeking a Minnesota-licensed Certified Occupational Therapy Assistant (COTA) to travel to patients’ homes, delivering one-on-one therapy under the supervision of an OT. You’ll build individualized plans, help with ADLs, and monitor progress in patients’ everyday environments.

Full-Time or Part-Time roles available. Compensation ranges from $82,000 to $110,000 per year (paid per visit) with mileage reimbursement, generous benefits, and strong clinical support.

Qualifications

• Active Minnesota COTA license and strong ADL knowledge.
• Ability to work independently in patients' homes with supervision as needed.
• Excellent communication and time-management skills.

Responsibilities

• Provide one-on-one Occupational Therapy treatments in patients' homes.
• Carry out individualized treatment plans established by the OT.
• Help patients improve activities of daily living.
• Collaborate with other clinicians and document progress.
• Monitor patient response to treatment and adjust plans accordingly.

Compensation: $82,000–$110,000 per year (Paid Per Visit) + Mileage Reimbursement

As a Certified Occupational Therapy Assistant with Home Health Care, Inc., you won't spend your entire day inside the same building waiting for the next patient to come through the door.

What You'll Do

Working under the direction of the supervising Occupational Therapist, you'll:

• Provide one-on-one Occupational Therapy treatments in patients' homes
• Carry out individualized treatment plans established by the OT
• Help patients improve activities of daily living
• Provide therapeutic activities and exercises
• Address upper-extremity strength, coordination, and functional use
• Teach adaptive and compensatory techniques
• Educate patients and caregivers
• Reinforce safe use of adaptive equipment
• Monitor patient response to treatment
• Recognize and communicate changes in patient status
• Report progress to the supervising Occupational Therapist
• Collaborate with PT, nursing, SLP, physicians, and interdisciplinary clinicians
• Complete timely and accurate clinical documentation
