Job summary

Home Health Care, Inc. is seeking a Certified Occupational Therapy Assistant (COTA) to deliver one-on-one in-home therapy under the supervising OT's plan of care.

You will help patients improve daily living skills, mobility, and upper-extremity function through individualized treatments in a home setting. Requires Minnesota COTA license, strong ADL rehab knowledge, and excellent communication with patients and caregivers.

Qualifications

• Active Minnesota COTA license.
• Strong understanding of ADLs and functional rehabilitation.
• Excellent patient and caregiver communication.
• Ability to independently organize your workday.
• Strong collaboration with supervising OTs.

Responsibilities

• Provide one-on-one Occupational Therapy treatments in patients' homes.
• Carry out treatment plans established by the supervising OT.
• Address ADLs, functional mobility, coordination, and upper-extremity function.
• Provide therapeutic activities and exercises.
• Teach adaptive and compensatory strategies.
• Educate patients and caregivers.
• Monitor progress and communicate changes to the OT.
• Collaborate with nurses, PTs, SLPs, physicians, and interdisciplinary clinicians.
• Complete timely clinical documentation.
