Job summary

Enhabit Home Health & Hospice is seeking a licensed therapy assistant to provide intermittent professional services to patients in the home setting. You will educate patients and families and collaborate with the health care team to implement the plan of care.

The role requires a license in the practicing state and at least one year of licensed therapy assistant experience, with CPR certification and reliable transportation. Medicare home health experience is preferred.

Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer therapy services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
