Job summary

Enhabit Home Health & Hospice is seeking a licensed Therapy Assistant to deliver intermittent professional therapy services. You will educate patients and families, and collaborate with the care team to follow the plan of care and physician orders.

The role requires graduation from an approved clinical education program and active licensure in the practicing state, with adherence to CPR and driving requirements for home visits.

Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.
• Licensed Therapy Assistant license required in the state of practice.

Responsibilities

• Administer therapy services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
