Job summary

Santè is seeking a Certified Occupational Therapist Assistant (COTA) to provide intermittent occupational therapy services in patients' residences in Chandler, AZ. You will work under an Occupational Therapist to support treatment planning, progress notes and communication with the care team.

New graduates with a COTA license are welcome. The role requires an Associate's degree in occupational therapy, CPR, and Level One Fingerprint Clearance; a minimum of one year SNF experience is preferred.

Qualifications

• Must have COTA license or be eligible for licensure in Arizona.
• Associate's degree in occupational therapy from accredited program.
• Minimum 1 year experience in Skilled Nursing setting preferred.
• Fingerprints clearance card Level One and CPR certification required.

Responsibilities

• Assist in treatment planning and implementation under OT supervision.
• Monitor client response and refer to OT for modifications.
