Job summary

Community Skilled Health Care Centre in Warren, OH is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to support residents in regaining independence and improving quality of life under the supervision of an Occupational Therapist.

You will implement therapy plans, document progress, and collaborate with nursing and therapy staff to deliver resident-centered care in a compassionate, team-oriented environment.

Qualifications

• Must hold current COTA license/certification.
• Graduate of an accredited OTA program.
• Experience in skilled nursing, long-term care, or rehabilitation preferred.
• Strong communication and organizational skills.
• Compassionate, dependable, and team-oriented.

Responsibilities

• Provide occupational therapy treatments according to established care plans and physician orders.
• Assist residents in improving functional abilities, mobility, independence, and quality of life.
• Implement therapeutic activities designed to improve daily living skills.
• Monitor resident progress and communicate changes with the Occupational Therapist and interdisciplinary team.
• Document treatments, resident responses, and progress accurately and timely.
• Collaborate with nursing, therapy, and other departments to support resident-centered care.
• Maintain compliance with facility policies, safety standards, and regulatory requirements.
