Job summary

North Park Post Acute in Tracy, California, is seeking a licensed Certified Occupational Therapist Assistant (COTA) to join a high-performing rehabilitation team. The role focuses on restoring independence and confidence in daily living within a collaborative Skilled Nursing environment.

Qualifications

• Must hold a current, active California Certified Occupational Therapy Assistant license.
• Graduated from an accredited OTA program.
• Must have NBCOT certification as an OTA or be NBCOT-eligible.
• Current CPR/BLS certification; renew as required by policy.

Responsibilities

• Implement OT treatment plans under supervision of a licensed OT.
• Deliver interventions for ADLs, IADLs, neuromuscular and cognitive skills.
• Monitor patient progress and communicate findings to the supervising OT.
• Assist in data collection for progress notes and care planning.
• Educate patients and caregivers on home programs and safe equipment use.
• Aid in transfers, positioning, and use of therapy equipment.
