Job summary

Lone Tree Post Acute in Antioch, CA is seeking a dedicated Certified Occupational Therapist Assistant (COTA) to join our in-house therapy team. In our 99-bed skilled nursing facility, you will help residents achieve independence and functional goals under supervision.

Responsibilities

• Deliver occupational therapy treatments under the direction of the Occupational Therapist.
• Collaborate with the interdisciplinary team to support resident goals.
• Educate residents, families, and caregivers on therapy programs and safe transitions home.
• Assist with discharge planning and continuity of care.
• Maintain accurate and timely documentation.
