Responsibilities

• Assess patient’s functional status and home environment to identify needs.
• Execute treatment plan and communicate with physicians.
• Instruct patients and caregivers on treatment and use of adaptive equipment.
• Maintain clinical records and coordinate with the care team.
• Participate in discharge planning and case conferences.
