Job summary

Trellis is hiring a Certified Occupational Therapy Assistant (COTA) in Chino, California. You will join our rehabilitation team and implement individualized treatment plans under a licensed OT to support quality patient care in a skilled nursing setting.

Qualifications

• Associate degree or higher in Occupational Therapy from an AOTA or ACOTE-accredited program.
• Current certification and good standing with applicable regulatory agencies and boards.
• Knowledge of resident rights and commitment to patient confidentiality.
• Ability to read, write, speak, and understand English effectively.
• Strong interpersonal skills and ability to maintain professional relationships.
• Demonstrated integrity and professionalism; adherence to Code of Conduct and compliance policies.
• Completion of all mandatory compliance and ethics training.

Responsibilities

• Implement and carry out patient treatment plans under supervision of a licensed Occupational Therapist.
• Communicate effectively and professionally with patients, families, physicians, and interdisciplinary team members.
• Provide timely, accurate written documentation in accordance with facility and departmental requirements.
• Report any changes in patient status or treatment needs to the supervising therapist promptly.
• Coordinate treatment goals with other members of the care team.
• Accurately prepare and submit patient charges to support appropriate billing.
• Support departmental operations, initiatives, and quality improvement efforts.
• Continue developing clinical and professional skills through ongoing education and training.
