Job summary

PACS Group is seeking a Certified Occupational Therapy Assistant (COTA) for Kern River Transitional Care Center on a full-time basis. You will work under an OT to deliver skilled treatments, enhance independence, safety, and mobility for residents, and participate in inter-disciplinary care planning.

Qualifications

• Associate degree from an accredited Occupational Therapy Assistant program.
• California COTA license in good standing.
• Experience in skilled nursing, long-term care, rehab, or post-acute care preferred.
• Proficiency with electronic health records and healthcare software.

Responsibilities

• Provide occupational therapy treatments as directed by supervising OT.
• Follow plans of care and report resident progress and changes in condition.
• Complete timely, accurate clinical documentation in accordance with policies.
• Maintain a safe treatment environment with infection control and safety protocols.
• Participate in resident care conferences and interdisciplinary team meetings.
• Assist with training Restorative Nursing Assistants.
• Communicate with the supervising OT regarding progress and discharge planning.
• Collaborate with physicians, nursing staff, and families.
• Educate residents and caregivers on treatment goals and home programs.
• Assist with adaptive equipment and safety devices as needed.
