Qualifications

• Must be licensed in the state where the SNF is located.
• Previous rehab SNF experience preferred.

Responsibilities

• Work with a multidisciplinary team to provide rehab care and individualized treatment plans.
• Deliver therapy in a progressive rehab environment addressing diseases and impairments.
• Provide in-house therapy with local management support.
