Job summary

Horizon Home Health Care is seeking a compassionate Certified Occupational Therapist Assistant (COTA) to join our home health team in Clark County, Ohio. Working under a licensed OT, you will help patients improve independence, safety, and quality of life at home.

Flexible, patient-centered care with meaningful one-on-one visits. Requirements include an active Ohio COTA license and CPR/BLS certification, plus a valid driver's license.

Qualifications

• Hold an active Ohio COTA license.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide occupational therapy treatments according to the established plan of care under the direction of a licensed OT
• Assist patients in improving their ability to perform activities of daily living and functional mobility
• Educate patients and caregivers on therapeutic exercises, adaptive techniques, and the safe use of equipment
• Observes, documents and reports patient progress and changes in condition to the supervising OT
• Maintains accurate and timely clinical documentation
• Collaborates with the interdisciplinary healthcare team to support optimal patient outcomes

We are looking for a compassionate and dependable COTA to join our home health team. Working under the supervision of a licensed OT, you'll provide individualized therapy services that help patients improve their independence, safety, and quality of life in their home.
