Job summary

Horizon Home Healthcare LLC is seeking a Certified Occupational Therapist Assistant (COTA) to provide home health OT services in Montgomery County, Ohio and surrounding areas. The role is part-time or PRN, with flexible scheduling and a focus on delivering patient-centered care in patients’ homes.

Responsibilities include implementing the plan of care, promoting independence in activities of daily living, and documenting progress while collaborating with the care team.

Qualifications

• Active Ohio COTA license.
• CPR and/or BLS certification.
• Valid driver's license with reliable transportation and proof of auto insurance.

• Collaborate with the interdisciplinary healthcare team to support patient outcomes.
