Job summary

Transition Care Telemetry Inc. is seeking a dedicated Certified Occupational Therapist Assistant to join our care team. The role involves assessing patients, developing treatment plans, and collaborating with physicians, nurses, and other therapists across home health, inpatient facilities, clinics, and SNFs.

You will document progress, educate families, and participate in continuing education to stay current with best practices in rehab, with an emphasis on compassionate, patient-centered care.

Qualifications

  • A degree in Certified Occupational Therapist Assistant from an accredited program.
  • Valid state licensure or eligibility for licensure as a COTA.
  • Strong knowledge of anatomy and the principles of rehabilitation.
  • Experience in various settings such as acute care, geriatrics, occupational health, SNFs, or home health is preferred.
  • Ability to work collaboratively within a team environment while also demonstrating independent decision-making skills.
  • Excellent communication skills to interact with patients, families, and healthcare professionals.
  • Familiarity with speech therapy techniques is a plus but not required.

Responsibilities

  • Conduct thorough assessments of patients' physical and cognitive abilities to determine their occupational therapy needs.
  • Develop and implement individualized treatment plans aimed at enhancing patients' ability to perform daily activities.
  • Provide therapeutic interventions including exercises and adaptive equipment recommendations.
  • Collaborate with physicians, nurses, and other therapists to coordinate patient care.
  • Document patient progress and modify treatment plans based on evaluations.
  • Educate patients and families on treatment goals and home exercise programs to promote independence.
  • Maintain knowledge of current best practices in occupational therapy and participate in continuing education.

Job Overview
