Job summary

CFG Health Systems LLC seeks an Licensed Social Worker / Professional Counselor to provide mental health care within a correctional facility in New Jersey. You will conduct intakes, assessments, and crisis interventions, delivering individual and group counseling while collaborating with a multidisciplinary team to plan and monitor individualized treatment plans.

Candidates must hold a Master’s degree in a behavioral health field and current licensure (LAC/LSW/LPC/LCSW) in NJ or applicable

Qualifications

• Master’s degree in social work, psychology, counseling, or related behavioral health field.
• Current and valid licensure as an LAC, LSW, LPC, or LCSW in the state where services are provided.

Responsibilities

• Conduct comprehensive mental health intakes, assessments, and crisis interventions.
• Provide individual and group counseling tailored to inmate/patient needs.
• Analyze psychosocial and behavioral factors to guide treatment planning and interventions.
• Develop, implement, and monitor individualized treatment plans with the treatment team.
• Maintain accurate progress notes, treatment documentation, and discharge summaries.
• Serve on the multidisciplinary treatment team and provide observational feedback.
• Refer inmates/patients to external services upon release to ensure continuity of care.
• Facilitate therapeutic group sessions and maintain a safe, therapeutic environment.
• Ensure compliance with facility policies, ethical standards, and state/federal regulations.
• Participate in ongoing quality improvement activities.
