Qualifications

• Five (5) years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public.
• Education and/or experience equivalent to 1. above. Completion of an approved six (6) month training program may substitute for six months experience; college study in sociology, psychology or behavioral science may substitute for credits; at least one year of the full-time experience described.
• Preference will be given to applicants with at least a high school diploma or its educational equivalent approved by a State’s Department of Education or a recognized accrediting organization.

Responsibilities

• Identify new HIV-positive patients using electronic health records and tracking tools to provide HIV education, adherence support, and linkage to care.
• Identify new hepatitis C-positive patients using electronic health records and tracking tools to provide hepatitis C education, adherence support, and linkage to care.
• Identify patients living with HIV to support continuation of antiretroviral medications, adherence, screenings, and reentry planning.
• Provide education, adherence support, and counseling for HIV PrEP/PEP eligible patients.
• Provide education and adherence support for patients at risk of HIV due to STIs or other conditions.
• Review antiretroviral medication administration lists and patient labs to identify adherence challenges.
• Review hepatitis C medication lists and labs to identify adherence challenges.
• Document all patient encounters in the electronic health records using Medspan templates.
• Obtain HIPAA medical record release forms to support care in jails and reentry planning.
• Collaborate with HIV and infectious disease specialists, discharge planning, and correctional health staff.
• Complete and update HIV and hepatitis C patient tracking databases.
• Participate in case conferences with infectious disease service and re-entry programs.
• Assist with research, analysis, patient tracking, quality improvement and implementation efforts.
• Attend required trainings and continuing education.
