VitalCore Health Strategies in Appleton, Wisconsin, is seeking a Full-Time Behavioral Health Professional (LCSW, LCPC, LMSW, LPC or equivalent) to join the Outagamie County Jail team.

The role provides crisis therapy, individual and group sessions, and collaboration with medical, psychiatry and community partners to deliver patient-centered care.

Qualifications

• Must be licensed master's level behavioral health clinician (LSW/LCPC/LMSW/LPC or equivalent).
• Minimum of one year social work experience in a public or private agency.
• Must pass a satisfactory background investigation.

Responsabilités

• Provide crisis stabilization, complete intakes and ongoing evaluations, and offer consultation to in-house and community services.
• Participate in monthly behavioral health calls; collaborate with medical team to determine best treatment.
• Perform risk assessments of inmates; coordinate care with interdisciplinary team including medical staff and community providers; discharge planning.
• Assist in planning and implementing the goals and objectives of programs and projects.
• Prepare documentation and reports; maintain accurate records.
• Utilize clinical skills such as prioritization and clinical judgment.
• Maintain excellent and timely documentation.
