The Group Home Supported Community Living Specialist (SCLS) is directly responsible to the Team Leader to which they are assigned and the Director of Residential Services.
This is a full-time position, Monday, Tuesday, Wednesday, 12pm-8pm and every other Saturday and Sunday 6am-8pm. This position qualifies for a hire-on bonus.
Responsibilities:
- Teach, train, and assist individuals with mental health issues in developing skills for independence in daily living activities, including:
- Coordinating health and medical needs.
- Training individuals to manage their own health and medical needs.
- Developing coping skills to improve mental illness symptoms.
- Maintaining skills for better community participation.
- Assisting individuals in accessing community services.
- Advocating for individuals’ rights and needs.
- Preparing individualized diet plans as needed.
- Writing Individualized Intervention Plans.
- Providing emergency/safety skills training.
- Training in managing personal business and property.
- Training in budgeting.
- Providing transportation (doctor’s appointments, church, etc.).
- Training in daily living skills.
- Orient new individuals in their home, assess their skills, and provide input for progress reports and Person-Centered Plans (PCP).
- Supervise individual goals and complete data collection.
- Supervise finances to guide budgeting for personal needs.
- Provide a supportive and therapeutic atmosphere.
- Supervise care and use of personal property.
- Train new staff on duties and agency policies.
- Follow the assigned work schedule.
- Never leave individuals receiving twenty-four-hour care unsupervised.
- Complete all records and documentation per established procedures.
- Attend staff meetings and training sessions.
- Ensure cleanliness, safety, and health standards in the residence.
- Stay informed of programming and administrative updates.
- Maintain confidentiality of all written and verbal information.
- Document behaviors and assist in writing intervention plans.
- Handle other duties as assigned by the Team Leader and Director of Residential Services.
- Monitor and document individual’s physician-ordered diet as needed.
- Assist individuals with daily living activities, including meal preparation, laundry, and housekeeping.
- Ensure client safety from environmental hazards.
Essential Functions:
- Provide quality care.
- Transport individuals.
- Provide training in daily living skills.
- Maintain housekeeping standards.
- Work with individuals who have behavioral problems, including aggression.
- Provide a supportive and therapeutic atmosphere.
- Administer medication and maintain accurate documentation.
- Exercise sound judgment in emergencies.
- Work according to the assigned schedule and remain alert during overnight shifts.
- Complete documentation according to agency requirements.
- Work independently to ensure the welfare and safety of individuals served.
- Comply with HIPAA regulations.
- Understand and comply with Chapter 57 rules and regulations.
- Participate in agency investigations as required.
Qualifications/Requirements:
- Must not have or be a carrier of serious infectious or communicable diseases.
- Must be at least 18 years old.
- High school diploma or GED preferred.
- Previous experience with individuals with mental health issues preferred.
- Experience in teaching skills related to cooking, cleaning, and personal hygiene preferred.
- Sound judgment in emergencies.
- Obtain and maintain a valid Iowa Chauffeur’s license.
- Successful completion of pre-employment physical, TB Tine, and drug screenings.
- Complete DIA approved medication management class.
- Willingness to work with individuals exhibiting behavioral disorders.
- Willingness to clean biological fluids/waste.
- Ability and desire to work as part of a team.
- Good verbal and written communication skills.
- Self-motivated.
- No legal convictions for assault, abuse, or bodily injury.
- Participate in DIA inspections as needed.
Physical Requirements:
- Ability to travel up to 70 miles.
- Ability to enter and exit homes that may not be universally accessible.
- Ability to perform laundry duties while carrying items.
- Ability to work a full scheduled shift.
- Ability to carry supplies weighing up to 50 pounds.
- Ability to provide physical care as needed.
- Ability to assist individuals with mobility.
- Ability to perform routine household tasks.
- Ability to assist individuals in a wheelchair.
- Ability to assist individuals in the shower or bath.
- Ability to carry purchases from car to house.
- Ability to perform snow removal as necessary.
