Job description

We Are Hiring
  • Department: Case Management
  • Schedule: Monday - Friday 8 a.m. - 5 p.m.
  • Hospital: Ascension Borgess - Pipp
  • Location: Plainwell and Kalamazoo, MI
What You Will Do

Coordinate and facilitate patient admission according to patient acuity, bed availability, and required services.

  • Perform onsite clinical assessments of potential candidates.
  • Coordinate transfer process, working with referral sources, insurance companies, and various healthcare staff and departments.
  • Monitor patient's insurance status during hospital stay.
  • Assist with development and improvement of processes designed to keep referral sources informed of patient status and discharge recommendations.
  • Provide pre-admission assessment, insurance verification, and pre-certification for admission.
What You Will Need

Licensure / Certification / Registration:

  • Social Worker credentialed from the Michigan Board of Social Work preferred. Any level accepted.
  • Registered Nurse credentialed from the Michigan Board of Nursing preferred.
  • Rehab Therapist preferred. Physical Therapist (PT), Occupational Therapist (OT), Certified Occupational Therapist Assistant (COTA), or Physical Therapist Assistant (PTA) accepted from respective Michigan Board of License.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
  • 3 years Critical Care experience.
  • Patient and family care centered experience.
  • Prior clinical education experience.
