Job summary

Qualifications

• RN license (PA) upon hire.
• Bachelor’s Degree in Nursing required.
• 5+ years of case management or discharge planning experience.
• Experience in pediatric or large acute-care hospital settings.
• Ability to work independently and in a team.

Responsibilities

• Assess patients and families for transition of care needs and develop, implement, and evaluate individualized discharge plans in collaboration with the interdisciplinary team.
• Initiate discharge planning upon admission and actively manage the progression of the discharge plan throughout the patient's hospitalization.
• Conduct formal discharge plan re-evaluations at least every eight (8) business days and more frequently when significant changes occur.
• Communicate approved days, denied days, authorization status, and payer determinations to support care progression and discharge planning activities.
• Collaborate with healthcare team members to identify and mitigate barriers that may delay discharge.
• Coordinate home care services, durable medical equipment, and other post-acute resources as appropriate to support safe discharge.
• Identify high-risk patients and develop targeted interventions to address identified risks.

Immediate need for a talented Coord Case Mgmt. This is a 06+months contract opportunity with long-term potential and is located in Philadelphia ,PA(Onsite).

Pay Range: $60 - $70/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

• 5+ years of relevant experience in case management or discharge planning.
• Technical Diploma in Nursing.
• Registered Nurse (RN) license in Pennsylvania upon hire.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and collaboratively within a team structure.
• Excellent problem-solving skills and customer service orientation.
• Proficient in Microsoft Word, Excel, and Access.
