Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Work with Director of Clinical Operations to share findings and support process improvement
• Maintain accurate documentation using electronic systems
