Job summary

Jukebox Health, Inc. is seeking a contracted In-Home Occupational Therapist in Lima, OH to help older adults and high-needs individuals live safely and independently at home. The role is remote-collaborative, with in-home visits and a minimum of 10 hours per week.

The therapist will conduct assessments, recommend home modifications, and educate clients on fall prevention. Comfort with new tech and driving are important; this role supports our mission to empower safer living at home.

Qualifications

• Must have OT degree from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.
• Strong interpersonal and clinical reasoning skills.
• Reliable transportation and valid driver’s license.
• Proficiency with technology and digital documentation.
• Ability to work independently while collaborating with a remote team.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives.
• Maintain accurate and timely documentation through our electronic system.
