Job summary

Jukebox Health is seeking a licensed Occupational Therapist to perform in‑home assessments and provide expert recommendations for home safety modifications. This contract role requires a minimum of 10 hours per week and includes reporting, education, and collaboration with a remote team.

Compensation is $120 per evaluation, with monthly payments. The position supports aging in place and independent living, serving clients nationwide with travel to client homes.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.
• Strong interpersonal and clinical reasoning skills.
• Reliable transportation and valid driver’s license.
• Proficiency with technology and digital documentation.
• Ability to work independently while collaborating with a remote team.

Responsibilities

• Conduct in‑home assessments and functional evaluations.
• Deliver personalized, evidence‑based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives.
• Maintain accurate and timely documentation through our electronic system.

More About Compensation

Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.

We pay per completed evaluation – this includes:

• Travel to/from client’s home
• The evaluation itself
• Documentation

If needed, we will pay additionally for extended travel – we will always discuss and approve ahead of time.
