Job summary

Jukebox Health, Inc. seeks an Occupational Therapist for in-home visits to support safer, more independent living for older adults and individuals with disabilities. This contracted role requires at least 10 hours per week and travel to clients' homes.

You will assess needs, recommend home modifications, and educate clients and caregivers on fall prevention and safe mobility, collaborating with a remote team while documenting outcomes electronically.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health or aging in place preferred.
• Ability to work independently while coordinating with a remote team.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work with Director of Clinical Operations to share findings and support process improvements.

We’re looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. Role Highlights

Key Responsibilities

• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver’s license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

More About Compensation

• Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.
• We pay per completed evaluation - this includes:
• Travel to/from client’s home
• The evaluation itself
• Documentation
• If needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
