Qualifications
- Active PT license (KY or compact) required.
- Minimum 1 year of PT experience in skilled nursing.
- Reliable transportation and consistent attendance required.
- Must be legally authorized to work in the United States.
Responsibilities
- Record patient data using an EMR and monitor vitals.
- Evaluate residents’ function, strength, balance, and mobility.
- Develop and implement individualized treatment plans.
- Provide therapeutic exercises and gait/balance training.
- Educate residents and caregivers on safety and fall prevention.
- Document evaluations and outcomes accurately.
- Maintain compliance with facility policies and standards.
