Job summary

Centra Healthcare is seeking a Physical Therapist (PT) to join a SNF / LTC team for a contract assignment in Estero, FL.

The ideal candidate will be licensed, have graduated from an accredited school, hold a CPR certification, and be prepared to develop and implement care plans while delivering skilled PT services per physician orders.

You will assess patient needs, adjust care plans to meet goals, and collaborate with all disciplines to support the patient’s recovery.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled PT services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals.
