Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist for a contract outpatient position in Sun City West, AZ. The PT will carry responsibility for planning and delivering skilled PT services in alignment with physician orders and organizational goals.

The role requires graduation from an accredited program, current state PT license, and CPR certification, with collaboration across disciplines to meet patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT).
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.

Responsibilities
