Job summary

AvaMed Workforce is seeking an Occupational Therapist to join a Skilled Nursing Facility in Williams, CA on a 6-month contract. You will assess residents, develop individualized treatment plans, and coordinate care with the interdisciplinary team.

Responsibilities include evaluations, interventions to improve mobility and ADLs, patient and family education, and precise documentation of progress. Travel stipends and optional health benefits may be available, with weekly pay.

Qualifications

• Active California OT license
• SNF experience preferred; new grads welcome

Responsibilities

• Conduct comprehensive OT evaluations and develop individualized treatment plans
• Provide therapeutic interventions to enhance functional mobility, ADLs, and cognitive/physical skills
• Educate patients, families, and caregivers on home programs and adaptive strategies
• Accurately document evaluations, daily notes, and progress reports in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care
• Maintain highest standards of professionalism, ethics, and clinical excellence

Education
