Job summary

Centra Healthcare Solutions Inc in Ohio is seeking an Occupational Therapist for a contract assignment in a SNF/LTC setting. The OT will assess patients, develop and implement therapy plans under physician orders, document progress, and educate patients and families in OT methods.

Requirements include graduation from an accredited OT program, current state OT license or active licensure application, CPR certification, and a 90% productivity standard.

Qualifications

• Must have graduated from an accredited OT program.
• Current state OT license or in process for licensure.
• Current CPR certification.
• Provide skilled OT services/interventions per physician orders.
• Assess patient needs, plan and modify care to meet goals.
• Collaborate with all disciplines to set team goals.
• Meet 90% productivity.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on capacity and tolerance.
• Document and record patient condition and progress.
• Educate patients and families in occupational therapy methods.
• Work under physician direction to achieve treatment goals.
