Job summary

Sweetwater Care is seeking a licensed Occupational Therapist in California to perform patient treatments, develop individualized plans, and educate patients on therapeutic exercises. The role requires collaboration with physicians and therapists and supervising assistants.

The OT will document progress, ensure regulatory compliance, and contribute to discharge planning while maintaining high-quality patient care and effective communication with families and care teams.

Qualifications

• Active state licensure required.
• Experience coordinating with therapists and other health professionals.
• Ability to develop individualized treatment plans.
• Experience documenting in electronic medical records per regulations.
• Ability to supervise assistants and students.

Responsibilities

• Consult with patients to assess condition and review referrals.
• Diagnose movement dysfunction and develop treatment plans.
• Provide therapy and teach exercises to improve mobility.
• Document progress and regulatory compliance in records.
• Supervise physical therapy assistants and rehab aides.
• Provide discharge summaries and communicate with families and other care providers.
• Participate in trainings and maintain policy adherence.
