Surve is South Florida’s premier concierge home health and wellness provider, delivering mindful, elevated care “With Purpose.” We cater to seniors and clients desiring both clinical and wellness services—ranging from skilled nursing to immersive mental health and therapeutic programming. We’re seeking a contractual Physician to lead clinical care delivery through telehealth and occasional in-home consultations.

Position Summary

As our MD, you’ll provide episodic clinical oversight and virtual care for Surve clients. Your principal mode of service will be telemedicine (video / phone), supplemented by occasional in-person home visits when clinically necessary. You’ll also guide clinical quality, support our nursing team, and help maintain regulatory compliance.

Key Responsibilities

• Deliver virtual medical consultations for episodic needs (e.g., care plan reviews, urgent evaluations, medication management)
• Conduct in-home appointments occasionally when telehealth isn’t sufficient
• Provide clinical backup and oversight when clients’ primary physicians are unavailable
• Serve on the Professional Advisory Committee (PAC) for policy and protocol development
• Act as CLIA medical director, if applicable, overseeing point-of-care testing and lab protocols (for any in-home services)
• Support Surve’s Director of Nursing and clinical team through telehealth case consultations, quality improvement, and patient safety discussions
• Help establish and refine clinical pathways, documentation standards, and medical guidelines
• Maintain consistent accessibility via telecommunication for urgent issues
• Optional: Provide telehealth primary or follow-up care when appropriate

Qualifications

• MD or DO with current, unrestricted Florida license
• Board certification preferred (Family Medicine, Internal Medicine, Geriatrics)
• Experience in virtual care, telehealth, or home health is highly desirable
• CLIA medical director experience or comfort supervising testing protocols
• Strong communicator adept at telehealth patient interactions
• Must carry malpractice insurance as an independent contractor
• Telehealth technology savviness and ability to conduct secure virtual visits
• Flexible, entrepreneurial mindset, comfortable in a start-up concierge model

Compensation & Schedule

• Paid per episode: Telehealth consults (e.g., care reviews, urgent virtual visits) and in-home visits are remunerated separately. Compensation depending on service type, complexity, and physician experience.
