Job summary

Sweetwater Care in California is seeking a licensed Vocational Nurse (LVN) to provide nursing care within scope of practice, following physician orders and facility policies. This role supports patient needs in a long-term care setting.

The ideal candidate has CA LVN licensure, is experienced with long-term care and Point Click Care charting, and may have prior nursing management exposure. Prioritize timely medication and treatment administration, and collaborate with physicians.

Qualifications

• Licensed as LVN in California (CA)
• One-year nursing management experience preferred
• Long-term care experience
• Point Click Care charting experience

Responsibilities

• Provide nursing care within scope of practice per physician orders and center policies
• Administer medications timely and according to facility policy
• Administer treatments timely and according to facility policy
• Receive, transcribe, and carry out physician orders as allowed by Nurse Practice Act
• Make rounds with physicians when necessary
• Educate families and patients
• Notify other departments of patient changes (room changes, LOA, admissions, discharges)
• Demonstrate infection control and participate in quality improvement activities
