Job summary

Mosaic is seeking Direct Care Workers to contract with us. This role involves visiting individuals in their homes and communities, coordinating visits, and delivering support according to each person’s needs and availability.

You will provide services including activities of daily living, meal planning and preparation, and transportation to appointments or social activities. Mosaic offers training and ongoing support to help you build a schedule that fits your lifestyle.

Qualifications

• Must be patient, compassionate, and have excellent communication skills.
• Must be able to work independently and follow a support plan to ensure consistent and appropriate support.
• Must be 18 years of age, have a valid driver’s license, and have a laptop or tablet with strong written/verbal communication skills.

Responsibilities

• Visit people needing services in their homes and communities.
• Coordinate visits based on individual needs and availability.
• Provide services in small groups or 1:1 with multiple visits per day as schedule allows.
• Assist with activities of daily living, meal planning and preparation, medication management, and transportation to appointments or social activities.
