Job description

Hourly Pay Range:

$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

  • Position: Continuum Care Manager
  • Location: Glenbrook, IL
  • Part Time: 24 hours per week
  • Hours: Saturday and Sunday, 7:00a-7:30p
  • Required Travel: Yes

What you will need:

  • Bachelor's degree required; Bachelor's degree in Nursing from an NLN-accredited school is preferred.
  • Minimum 2 years clinical nursing experience preferred.
  • Minimum two (2) years of utilization review, discharge planning, case management, or disease management preferred.
  • Knowledge of InterQual or MCG criteria is preferred.
  • Clinical certification, such as case management certification, is beneficial.
  • Basic Life Support (BLS) for Healthcare Providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
  • Ability to communicate and work collaboratively with stakeholders and team members.
  • Knowledge of community resources.
  • Experience with Microsoft Office Suite.
  • Strong interpersonal, oral communication, and data entry skills.
  • Experience with Electronic Medical Record (EMR) platforms preferred.
  • Proven leadership skills.
  • Ability to work independently, prioritize, and coordinate care plans efficiently.

What you will do:

  • Provide comprehensive case management and utilization management services, including discharge planning, across various clinical areas.
  • Collaborate with social work to identify and address care transition gaps. Tasks include admission review, stay reviews, liaising with insurance, referring to Physician Advisor, and collecting QA data.
  • Work as part of a collaborative, interdisciplinary team, utilizing assessment, implementation, coordination, monitoring, and evaluation skills.

The Case Manager is responsible for a designated patient caseload, planning effectively to meet patient needs, manage length of stay, and promote resource utilization.
