Qualifications

    We are currently accepting applications for Homecare Private Duty RNs. Both part time and full time schedules; varied hours are available. Choose where you want to work! Multiple locations available around Columbus:

    Provides the direct continuous nursing care or skilled nursing visits for individual and/or groups of acute, chronically ill, and terminally ill patients in the home setting as prescribed by NCH and community physicians in compliance with regulations as established by the State Board of Nursing, Medicare / Medicaid guidelines, JC and the policies of NCHH and NCH. The care provided by the RN shall reflect the highest standards of nursing practice and shall be provided in a manner which promotes quality, continuity and safe patient care. Responsible for providing care for pediatric patients and select adult patients. Demonstrates competencies defined (*) in RN responsibilities in providing age-appropriate care to patients from the age of birth through 21 years, and select adult patients.

    Responsibilities

    1. * Follows the patient plan of care, as ordered, authorized and scheduled, under the direction of the Patient Care Coordinator (PCC) (RN).

    2. Develop/update plan of care/document, document and communicate changes/updates, update information system, and participate in care planning.

    3. Develop/finalize/revise/update/terminate service agreements as appropriate.

    4. * Promotes and provides necessary patient/caregiver patient education for prescribed therapies.

    5. * Implements continuous nursing care or skilled nursing visits for pediatric patients/families in sometimes stressful environment, demonstrating competence in nursing care according to established standards and protocols.

    6. * Implements the individualized plan of care for each patient (and/or in a group setting), which states the patient outcomes, goals and means by which these goals will be achieved.

    7. *Collaborates with the patient/family in order to develop an individualized care plan using standards, protocols, and current nursing literature and research.

    8. *Correctly calculates, prepares, and administers medications by the five rights of medication administration.

    9. Initiates purposeful, written and verbal communication with patient/family, and other members of the health care team to convey nursing actions, patient/family response, and progress toward goals during routine as well as stressful situations.

    10. Consistently uses independent decision making appropriate for meeting patient/family needs.

    11. *Implements home continuous nursing care or skilled nursing visits per established policies, procedures, and physician's orders.

    12. *Ensures that appropriate patient care protocols and quality controls are in place for the delivery, training, reporting, follow-up and evaluation of home nursing services and/or infusion services.

    13. Possess advanced ability to anticipate, manage, and evaluate patient needs, and serve as expert resource to other personnel.

    14. Assure complete documentation for billing purposes and match practice with authorized allowables with patient needs.

    15. Provide initial/ongoing advocacy, customer service, and communication with family and all customers

    16. Need/ability to balance cost, quality, and mission.

    17. Maintains confidentiality of all patient/agency information.

    18. Assumes responsibility for continued professional growth.

    19. Possess knowledge of medical terminology, office equipment, different service lines/roles, documentation requirements, information system, utilization review, referral process/sources, discharge process, care planning, use of internal/external resources, applicable regulatory requirements, community resources, chart process, scheduling, resource availability, payer coverage policies, medical equipment, ability to provide patient education as appropriate to practice, and basic knowledge of diagnoses and procedural coding, as appropriate.

    20. Performs basic skills in the following: phone communication, computer skills (word processing and data entry), communication, organizational/multi-tasking, customer service, team building, supervising staff as appropriate, management/leadership skills as appropriate, negotiation, conflict management, decision making, and diplomacy skills.

    21. Performs other duties as assigned.

    MINIMUM QUALIFICATIONS:

  • Must be a graduate of an accredited school of registered nursing, ACEN or CCNE.
  • Must be licensed as a Registered Nurse in the State of Ohio.
  • One year previous pediatric home health experience preferred.
  • CPR certification required.
  • Strong knowledge of family-centered care.
  • Ability to work independently and react appropriately to crisis situations.
  • Must have experience and knowledge in the care of medically fragile children, specifically G/J tube care and tracheostomy care and a beginning knowledge of ventilator care and maintenance.
  • Must have the ability to function collaboratively within the framework of an Interdisciplinary Team.
  • Home Visit RN

    1. Strong problem-solving, organization, leadership, and communication skills.

    2. Proficiency in IV starts, central line care, and infusion therapy required.
